Oversee 25 patients who are receiving full time care 🤌
4-6 weeks in-person training in Crown Heights and then remote
Position Overview:
☎ check in with patients over the phone
🤝 de-escalate when patients are dissatisfied
📝 complete paperwork by deadlines
🛌 conduct 8 home visits monthly around NY - MUST BE COMFORTABLE GOING INTO PATIENTS HOMES
🎓 BA or Associates required!
