Patient Case Manager

What You'll Do:- Patient Outreach - Connect with patients who left or are considering leaving- Retention & Recovery - Convince patients to stay or return to our agency- Issue Resolution - Deescalate conflicts and troubleshoot service problems

• Coordination Follow-up - Work with teams to resume tailored services
• Target Achievement - Hit weekly retention targets
